Service Disclaimer

At Gentle Wax of Sarasota, we have a strict policy against inappropriate behavior while receiving any service. If you wouldn’t do it at your doctor’s office, do not do it here. We strive to make everyone comfortable during their service, and are providing our clients with a professional service. We require all clients respect us in our profession.

Gentle Wax of Sarasota reserves the right to refuse service to anyone at any time, for any reason the staff may see fit. Any inappropriate behavior will not be tolerated. If you are acting in any way we see as inappropriate, the service will be stopped, and you will be asked to leave. You are financially liable for any fees associated with your service.

Consent To Perform Services

Please note that waxing does have certain side effects such as skin removal, redness, swelling, tenderness, etc.

There are certain medications that are contraindications for waxing, and may cause increased skin sensitivity, skin lifting (top layer of skin is removed), redness, swelling, tenderness, or scabbing. We do not recommend getting waxed if you are currently taking any of the following: Antibiotics, Adapalene (Acne medication), Alustra (Retin A), Avage (Tazorac – Acne medication), Avita (type of Retin A), Clindamycin, Differin (Acne medication), Doxycycline, Erythromycin, Isotretinoin (like Accutane), Madifloxicine, Metronidazole, Minocycline, Renova (Retin A), Tazarac (Acne medication), Tazarotene (like Tazorac), Tetracycline, Tretinoin (like Retin A)

If you are being treated for diabetes or cancer, we recommend that you get approval from your doctor before getting waxed, as some treatments may cause skin sensitivities. Please wait 7 days to get waxed after getting a chemical peel or microdermabrasion, and 48 hours to get waxed after using products with Alpha Hydroxy Acid (AHA), or Glycolic Acid in the area you will be waxing. Please wait for any overly exposed/sunburnt skin to fully heal before getting waxed.

I have read the above information and if I have any concerns, I will address these with my esthetician. I give permission to my service provider to perform the waxing procedure we have discussed, and will hold all staff harmless from any liability that may result from this treatment. I have given an accurate account of the questions asked above including all known allergies or prescription drugs or products I am currently ingesting or using topically. I understand my service provider will take every precaution to minimize or eliminate negative reactions as much as possible.

I have read and understand the post-treatment home care instructions. I am willing to follow recommendations made by my service provider for a home care regimen that can minimize or eliminate possible negative reactions. In the event that I may have additional questions or concerns regarding my treatment or suggested home product / post-treatment care, I will consult the service provider immediately.

I agree that this constitutes full disclosure, and that it supersedes any previous verbal or written disclosures. I certify that I have read, and fully understand the above paragraphs and that I have had sufficient opportunity for discussion to have any questions answered. I understand the procedure and accept the risks. I do not hold the service provider responsible for any of my conditions that were present, but not disclosed at the time of this skin care procedure, which may be affected by the treatment performed today.

I am of 18 years of age, or have parental consent to receive these services from Gentle Wax of Sarasota.

The Service is controlled, operated and administered by La Jolie from our offices within the USA. If you access the Service from a location outside the USA, you are responsible for compliance with all local laws. Indemnification

Termination/access restriction

Gentle Wax of Sarasota reserves the right, in its sole discretion, to terminate your access to the Site and the related services or any portion thereof at any time, without notice. To the maximum extent permitted by law, this agreement is governed by the laws of the State of Nevada and you hereby consent to the exclusive jurisdiction and venue of courts in Nevada in all disputes arising out of or relating to the use of the Site. Use of the Site is unauthorized in any jurisdiction that does not give effect to all provisions of these Terms, including, without limitation, this section.

You agree that no joint venture, partnership, employment, or agency relationship exists between you and Gentle Wax of Sarasota as a result of this agreement or use of the Site. Gentle Wax of Sarasota’s performance of this agreement is subject to existing laws and legal process, and nothing contained in this agreement is in derogation of Gentle Wax of Sarasota right to comply with governmental, court and law enforcement requests or requirements relating to your use of the Site or information provided to or gathered by Gentle Wax of Sarasota with respect to such use. If any part of this agreement is determined to be invalid or unenforceable pursuant to applicable law including, but not limited to, the warranty disclaimers and liability limitations set forth above, then the invalid or unenforceable provision will be deemed superseded by a valid, enforceable provision that most closely matches the intent of the original provision and the remainder of the agreement shall continue in effect.

Unless otherwise specified herein, this agreement constitutes the entire agreement between the user and Gentle Wax of Sarasota with respect to the Site and it supersedes all prior or contemporaneous communications and proposals, whether electronic, oral or written, between the user and Gentle Wax of Sarasota with respect to the Site. A printed version of this agreement and of any notice given in electronic form shall be admissible in judicial or administrative proceedings based upon or relating to this agreement to the same extent an d subject to the same conditions as other business documents and records originally generated and maintained in printed form. It is the express wish to the parties that this agreement and all related documents be written in English.

Changes to Terms

Gentle Wax of Sarasota reserves the right, in its sole discretion, to change the Terms under which Products, Services & waxapp.gentlewax.com is offered.
